New Passport for Children

Children are not endorsed on Parent's Passports and need to apply for separate passports.

Requirements:

Submit Registration of birth form S-I. (Visit Registration of Birth page)
Complete Form-A
4 current passport size photographs with front view and white background.
Copy of birth certificate.
Copy of father’s Pakistani passport or NICOP/CNIC.
NICOP application. (Visit NICOP page)
Fee in the shape of money order or cashier’s check payable to the Consulate General of Pakistan. Personal checks are not acceptable and fees are non refundable.
Prepaid self addressed return envelope for applications sent by mail (US Postal Service with tracking number is preferable).

Note: Passport with one-year validity will be issued in the absence of NICOP/CNIC. The applicant must submit NICOP application with the passport application. The passport will be extended to full five years validity on obtaining NICOP without any further charge
