Requirements:
- For
documents sent by mail the signature of the applicant/s on the power of
attorney/authority letter must be notarized. Notarization is not required if
the person/s signing the document is/are bringing it to the Consulate in
person.
- Copy
of Pakistani passport or CNIC/NICOP.
- If
the person signing the power of attorney/authority letter is not a Pakistani
national or of Pakistani origin, then the document needs to be attested
first from the US Department of State and then submitted for Consulate’s
attestation.
- If
the document has been issued by any agency or authority of the United States
or is any other local document then it needs to be first attested by the US
Department of State and then submitted for the Consulate’s
attestation.
- Fee
in the shape of money order or cashier’s cheques payable to the Consulate
General of Pakistan. No cash or personal cheques are acceptable. Fees are
non refundable.
- Prepaid
self-addressed return envelope for applications sent by mail. US Postal
Service with tracking number is preferable.
- Please
attach each additional copy of all documents for Consulate’s record.
